No, you buy the band only. The pictures show the band on an Apple Watch.

Description
The extension is made of the same strong silicone as the Ocean Band and makes the 49 mm Ocean Band 50 mm longer – perfect if you want to wear the watch over a wetsuit.
It has an adjustable loop in corrosion-resistant titanium that you attach anywhere on the band. The extension is used together with a regular Ocean Band.
